Location: Tuba City, Arizona
Schedule: 8:00AM – 5:00PM x 5
Term: 12-week assignment with the potential for extension
Pay Rate: $65 – $70/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a PT for a temporary assignment in Tuba City, Arizona. These positions are 12-week contracts with the ability to extend.
Responsibilities for a PT:
• Provide quality physical therapy services to a predominately Native American population for the treatment or prevention of physical disability or disease
• Manage and implement the Physical Therapy program
• Conduct physical therapy evaluations and treatment of inpatients (regular work week and weekend rotation) and outpatients, with ages ranging from birth through geriatric
• Provide specialty services such as wound care, prosthetic and orthotic clinic, geriatric clinic, high school clinic, functional clinic, spinal cord clinic, assistive technology/adaptive equipment evaluation and treatment, and Durable Medical Equipment Prosthetic Orthotic Supply Program
• Assume direction and development of the physical therapy section, including continuing quality improvement, risk management, and safety
• Develop and maintain physical therapy policies and procedures
• Participate in Joint Commission preparation activities
• Conduct staff education, reporting, staff meetings, and in-service training
• Fulfill responsibilities listed on the Medical Staff Credentialing Core Privileges form
• Maintain a valid, current, full, and unrestricted physical therapy license in any U.S. state, District of Columbia, or Commonwealth of Puerto Rico
• Maintain a current Basic Life Support (BLS) certification by the American Heart Association
• Meet requirements to be credentialed to serve on the Medical Staff and provide all related credentialing information
• Submit fingerprinting for clearance prior to start
• Complete orientation requirements as scheduled
