Schedule: 8:00AM – 5:00PM x 5
Term: 13-week assignment with the potential for extension
Pay Rate: $62 – $67/hr

Astrya Global, a San Diego based medical staffing agency, is hiring a PT for a temporary assignment in Rutland, Vermont. These positions are 13-week contracts with the ability to extend.

Responsibilities for a PT:

• Actively monitors and amends patient visit schedule to maximize productivity and patient need, including calling patients to reschedule visits, reshuffling patient load, and communicating recommended changes to supervisor
• Performs comprehensive assessments (OASIS and PT) of assigned patients including psychosocial, educational, safety, and risk factor parameters in accordance with federal and state regulations and APTA guidelines
• Develops individualized plans of care to promote optimal functional independence and safety, creating patient-specific goals and ensuring logical interventions and advancement toward those goals
• Plans and provides skilled physical therapy treatment including therapeutic techniques and patient/caregiver education, assessing patient status and progress on each visit
• Coordinates patient care with other clinical team members including physicians, nurses, caregivers, other rehab services, and students as Care Manager and/or team member
• Supervises and directs clinical activities of PTAs, PT students, and home care aides in accordance with federal and state regulations and APTA guidelines
• Completes and coordinates required clinical documentation to reflect care provided and outcomes achieved in accordance with federal and state regulations and APTA guidelines
• Completes required paperwork to ensure accurate charging and billing processes, addresses inconsistencies in insurance coverage, and notifies supervisor of issues in a timely manner
• Participates in in-service training programs for customers including Rehabilitation, Medical, Nursing staff, volunteers, and community groups
• Maintains and enhances personal knowledge and promotes standards in physical therapy
• Proactively engages in marketing and promotion of Rehab services including developing specialized areas of service and participating in public health fairs and medical community events
• Carries out patient-related health assessments including PT/INR, blood pressure, and other applicable assessments
• Carries out patient-related procedures including staple/suture removal, wound care, and other applicable procedures
• Communicates changes in patient care schedules to scheduler and supervisor in a timely manner
• Completes OASIS time points in accordance with agency, state, and federal guidelines, regulations, and policies
• Works collaboratively with QI staff to maximize accuracy of clinical notes and OASIS documents
• Follows agency protocol with McKesson processes including locking-down visits, follow-up notes, and visit completion
• Accurately and completely fills out required agency, state, and federal forms in a timely manner and ensures forms are readily available during visits
• Carries out visits in a time-effective manner, ensuring supervisory visits are not combined with routine visits when necessary
• Assesses patients’ needs for HCA services, develops and coordinates plans of care with patient/caregiver, and provides ongoing supervision in accordance with state and federal regulations
• Coordinates safe patient discharges and/or transfers and ensures all appropriate paperwork and patient information is processed in accordance with department policies and state and federal regulations
• Transmits patient incident reports including infections and injuries to QI Specialist in a timely manner
• Participates in regular peer review audits, provides constructive feedback, and openly accepts feedback in a professional manner
