Astrya Global, a San Diego based medical staffing agency, is hiring a PHARM for a permanent assignment in Sitka, Alaska.
Responsibilities for a PHARM:
• Review all prescriptions for appropriateness of therapy, legality, completeness, contraindications, interactions, allergies, adverse drug reactions, lab value abnormalities, and follow up by screening patient charts
• Provide clinical consultation and clarification, resolving any concerns or questions with prescribers prior to dispensing
• Document all clinical activities and interventions accurately and completely
• Enter prescriptions into pharmacy software
• Verify finished prescription orders in a timely manner while maintaining all required record keeping and documentation
• Work independently with minimal supervision, organizing and prioritizing work assignments
• Advise patients clearly and accurately regarding their medications, appropriate outcomes, indication, drug name and strength, usage instructions, and expected changes
• Communicate regularly with patients and healthcare providers to resolve problems and provide optimal care
• Perform chart reviews for prescription refills and medication therapy management
• Write recommendations for providers and request prescription refills on the patient’s behalf
• Provide drug information, drug therapy consultation, and staff education relating to drug therapy
• Direct pharmacy support personnel and verify the daily activities of pharmacy technicians
• Serve as a role model and educator for pharmacy students and technicians
• Participate in clinical and administrative activities including quality control and quality improvement within the department and clinic
• Attend meetings and participate on committees as requested
• Assure availability, preparation, and control of medications and select appropriate drugs for formulary
• Work in pharmacy-run clinics and take on additional departmental duties and responsibilities as assigned
• Provide health promotion and disease prevention activities related to drug use and preventative drug therapy
• Support Long-Term Care consultant pharmacist functions including medication regimen reviews, regulatory compliance activities, quality improvement initiatives, and survey preparation
• Assist with evaluation and improvement of medication-use systems in the Long-Term Care setting, including identification of high-risk medication practices, psychotropic stewardship, controlled substance oversight, and medication safety initiatives
• Participate in interdisciplinary care planning and provide medication-related education to facility staff
• Support quality assurance and performance improvement activities related to Long-Term Care services
• Maintain certification in Basic Life Support (BLS) for Healthcare Providers
• Complete Clinical Competency assessment within 3 months of hire and every three years
• Complete High Risk Competency assessment within 3 months of hire and annually
• Maintain an unrestricted license as a registered pharmacist
• Travel up to 10% of the time as required
