Astrya Global, a San Diego based medical staffing agency, is hiring a PHARM for a permanent assignment in Milford, Delaware.
Responsibilities for a PHARM:
• Establish a collaborative practice program with oncologists within identified areas of need including supportive care measures for patients receiving chemotherapy on an inpatient and outpatient basis
• Conduct staff development programs or educational services to pharmacy staff, nursing staff, oncologists, and other healthcare professionals
• Serve as a subject matter expert in oncology, completing drug information questions for the oncology service line and reviewing Electronic Health Record care plans to ensure consistency with current national guidelines
• Round with Oncology Fellows, Nurse Practitioners, and Physicians to optimize therapy according to established national evidence-based guidelines, including recommendations regarding correct medications, monitoring outcomes, dose adjustments, and supportive care measures
• Provide patient-specific medication education using the teach-back method for new oral oncolytics and intravenous chemotherapy as needed
• Review oncology care plans within the Electronic Health Record, including new care plans and continual review of existing care plans for updates and appropriateness
• Identify and evaluate medication-related problems based on clinical presentation, available history, or laboratory data
• Serve as a preceptor to pharmacy residents and students
• Actively participate in Oncology Service Line meetings to work towards improving safety, efficacy, and quality of cancer care
• Promote and demonstrate rational, cost-effective drug therapy with a focus on cost avoidance and cost savings within the Oncology Service Line
• Perform all other duties as assigned within the scope and range of job responsibilities
