Job Title: Admin
Location: Sunnyvale, California
Schedule: 7:00AM – 3:00PM x 5
Term: 13-week assignment with the potential for extension
Pay Rate: $55 – $60/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a Admin for a temporary assignment in Sunnyvale, California. These positions are 13-week contracts with the ability to extend.
Responsibilities for a Admin:
• Admit patients to hospice service
• Assess patient and family physical, psychosocial, environmental, safety, and developmental needs
• Develop individualized plans of care based on patient assessments
• Document observations and interventions related to the patient’s environment
• Coordinate care with a multidisciplinary team
• Participate in the hospice program’s quality assessment performance improvement program
• Conduct follow-up visits as needed
• Provide care services within assigned territory including San Francisco County, San Mateo County, and Santa Clara County
• Maintain a minimum of one year of hospice experience
