Job Title: Home Health
Location: Le Mars, Iowa
Schedule: 7:00AM – 7:00PM x 3
Term: 13-week assignment with the potential for extension
Pay Rate: $52 – $56/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a Home Health for a temporary assignment in Le Mars, Iowa. These positions are 13-week contracts with the ability to extend.
Responsibilities for a Home Health:
• Provide nursing care to patients in their home setting
• Perform OASIS assessments for in-home patients
• Develop and manage care plans for in-home patients
• Utilize Epic electronic health record system for documentation
• Participate in the on-call rotation for the department (one in four nights, weekends, and holidays)
• Work scheduled day shifts, Monday through Friday
• Maintain valid RN licensure
• Maintain a minimum of 2 years of experience performing OASIS assessments and care planning
