Job Title: Home Health
Location: Grand Junction, Colorado
Schedule: 4:00PM – 4:00AM x 3
Term: 13-week assignment with the potential for extension
Pay Rate: $49 – $53/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a Home Health for a temporary assignment in Grand Junction, Colorado. These positions are 13-week contracts with the ability to extend.
Responsibilities for a Home Health:
• Perform comprehensive patient assessments
• Develop and update individualized plans of care
• Provide skilled nursing care and appropriate interventions
• Monitor patient progress and report changes to the care team
• Educate patients and families on medications, treatments, and care plans
• Coordinate care with LPNs, therapists, caregivers, and providers
• Complete timely and accurate documentation in EMR
• Participate in on-call rotation
• Conduct independent visits in patient homes
• Maintain active Registered Nurse (RN) license in the State of Colorado or active NLC compact license
• Maintain a valid driver’s license, reliable transportation, and auto insurance
• Demonstrate strong assessment and critical thinking skills
• Manage a structured caseload independently
• Utilize EMR systems for documentation (HCHB preferred)
• Collaborate across a supportive, interdisciplinary team
