Job Title: Home Health-Hospice
Location: Newport, Vermont
Schedule: 7:00AM – 5:00PM x 4
Term: 13-week assignment with the potential for extension
Pay Rate: $55 – $60/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a Home Health-Hospice for a temporary assignment in Newport, Vermont. These positions are 13-week contracts with the ability to extend.
Responsibilities for a Home Health-Hospice:
• Work Monday to Friday 8 am to 5 pm with flexibility based on patient needs
• Scheduled for 1 weekend a month, followed by 2 days off (1 day prior to Saturday and 1 day after Sunday)
• Available for emergency-based on-call scheduling as needed
• Utilize OASIS (Outcome and Assessment Information Set) for patient assessments
• Provide home health and hospice care services
• Travel from patient to patient for home visits
• Manage and adjust to changes in schedule as required
• Maintain a valid driver’s license in good standing
• Maintain current BLS certification
• Utilize HealthWyse (Careficient) EMR system for documentation
• Deliver outpatient care services
• Comply with background check requirements
