Job Title: Primary Care Physician

Schedule: 8:00AM – 5:00PM x 5
Term: 26-week assignment with the potential for extension.
Pay Rate: $136 – $146/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a Primary Care Physician for a temporary assignment in San Jose, California. These positions are 26-week locum tenens contracts with the ability to extend.
Responsibilities for a Primary Care Physician:
• Board certified or non-boarded with appropriate experience
• Download and complete the required Provider Questionnaire form prior to submitting
• Meet all qualifications, privileging, and compliance documentation requirements as determined by the client
• Ensure all credentialing and privileging requirements are fulfilled prior to performance of services
