Job Title: Home Health
Location: Battle Creek, Michigan
Schedule: 8:00AM – 5:00PM x 5
Term: 16-week assignment with the potential for extension
Pay Rate: $46 – $49/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a Home Health for a temporary assignment in Battle Creek, Michigan. These positions are 16-week contracts with the ability to extend.
Responsibilities for a Home Health:
• Triage emergent situations and apply advanced nursing knowledge independently
• Monitor acute and chronic medical conditions in coordination with Primary Care Providers (PCPs)
• Utilize care plans independently to provide clinic and home-based direct care
• Administer treatments, procedures, and medications as prescribed to participants
• Perform wound care and treatments as prescribed in the participant’s plan of care
• Administer medications, immunizations, and other care as prescribed
• Perform assessments and actively participate in care plan development
• Make regular home visits to provide care and treatment as ordered by the PCP, including wound care, lab draws, nurse assessments, and other nursing tasks
• Perform on-call duties and determine immediate needs for participants as needed
• Work beyond normal working hours and on weekends and holidays as necessary
• Complete all required education and employee file requirements on time
• Document in the participant’s medical record as required by policy
• Collaborate with staff, participants, and participant families using a participant-centered care model
• Keep leadership informed of problems or issues
• Perform other duties as assigned
