Job Title: Adult-Gerontology Nurse Practitioner (AGNP)
Location: Escondido, California
Schedule: 8:00AM – 5:00PM x 5
Term: 26-week assignment with the potential for extension
Pay Rate: $111 – $120/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a Adult-Gerontology Nurse Practitioner (AGNP) for a temporary assignment in Escondido, California. These positions are 26-week contracts with the ability to extend.
Responsibilities for a Adult-Gerontology Nurse Practitioner (AGNP):
• Provide home-based hospice and palliative care as a Nurse Practitioner for patients with serious or life-limiting illnesses
• Conduct comprehensive patient assessments and develop individualized care plans
• Manage symptoms for patients receiving hospice and palliative care services
• Perform home visits to patients within the service area
• Facilitate goals-of-care discussions and advance care planning, including POLST and advance directives
• Provide patient and family education related to diagnosis, care, and end-of-life planning
• Coordinate care with the interdisciplinary team and community physicians
• Conduct approximately 45–50 patient visits per month
• Participate in rotating weekend coverage and occasional on-call duties as part of 24/7 patient coverage
• Document all patient encounters accurately and in a timely manner using the My Unity EMR system
• Participate in Interdisciplinary Team (IDT) meetings and quality improvement initiatives
• Collaborate closely with the Hospice & Palliative Care Medical Director
• Serve as a resource and support for home health aides and the care team
• Maintain valid California NP and RN licenses, DEA, NPI, BLS, and AANP/ANCC certification
• Hold or obtain ACHPN certification within one year of hire
• Maintain a minimum of 3 years of hospice or palliative care experience
• Maintain a valid California driver’s license and auto insurance for local travel to patient homes
• Spanish bilingual skills are a plus
