Schedule: 8:00AM – 5:00PM x 5
Term: 13-week assignment with the potential for extension
Pay Rate: $62 – $67/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a PT for a temporary assignment in San Ramon, California. These positions are 13-week contracts with the ability to extend.
Responsibilities for a PT:
• Provide physical therapy services in assigned county territories including Area #4 (Danville, Martinez, Moraga, Alamo, Clayton, Concord, Lafayette, Orinda, Pleasant Hill, Walnut Creek) and Area #5 (Pittsburg, Discovery Bay, Byron, Antioch, Brentwood, Oakley, Bethel Island, Bay Point)
• Travel outside assigned areas as required to maintain weekly visit expectations
• Provide weekend coverage as required
• Meet productivity requirements of at least 27 points per week if under a guarantee arrangement
• Complete OASIS ReCertification, OASIS RoC, Routine, Eval, Discharge, Eval SoC, Eval RoC, Eval ReCert, Eval F/up, Re-Eval, Eval Discharge, Oasis Discharge, and Oasis F/up visits as assigned
• Maintain accurate documentation within required timeframes
• Manage drive/travel time, visit time, documentation time, and phone calls to patients and physicians
• Be willing to see Covid+ and Covid PUI patients
• Maintain current California PT License, BLS certification, and valid Driver’s License
• Maintain required immunizations and health credentials including TB (annual), MMR, Varicella, HepB, TDap (within 10 years), and Flu (annual)
• Comply with auto insurance and vehicle registration requirements
• Complete required drug screen and physical examination
• Disclose all Requested Time Off (RTO) at the time of submission
