Job Title: Case Manager
Location: Greenbrae, California
Schedule: 8:00AM – 5:00PM x 5
Term: 13-week assignment with the potential for extension
Pay Rate: $98 – $105/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a Case Manager for a temporary assignment in Greenbrae, California. These positions are 13-week contracts with the ability to extend.
Responsibilities for a Case Manager:
• Provide leadership and administrative oversight in Case Management, Care Coordination, and Utilization Management
• Lead interdisciplinary clinical and operational programs and staff
• Oversee discharge planning, patient progression, and transitions of care in an acute-care environment
• Drive performance improvement initiatives across care coordination functions
• Ensure regulatory and accreditation readiness
• Apply advanced knowledge of case management, utilization management, and care coordination practices
• Maintain BSN certification; master’s degree in Nursing, Health Administration, or Business Administration preferred
• Maintain Case Management certification (e.g., CCM or ACM) preferred
• Demonstrate a minimum of five (5) years of experience in an acute healthcare setting
• Demonstrate a minimum of five (5) years of progressively responsible leadership experience in a related function
