Location: Grand Junction, Colorado
Schedule: 8:00AM – 5:00PM x 5
Term: 13-week assignment with the potential for extension
Pay Rate: $55 – $60/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a PT for a temporary assignment in Grand Junction, Colorado. These positions are 13-week contracts with the ability to extend.
Responsibilities for a PT:
• Perform assigned physical therapy visits, including evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits
• Develop and implement individualized physical therapy plans of care based on patient needs and physician orders
• Provide skilled interventions such as gait training, balance training, therapeutic exercise, mobility training, and fall prevention
• Educate patients and caregivers on treatment plans, home exercise programs, and safety strategies
• Communicate patient progress, concerns, or changes in condition to the interdisciplinary care team
• Collaborate in the clinical oversight of Physical Therapist Assistants (PTAs), as applicable and permitted under Colorado law and agency policy
• Complete all required documentation accurately and timely in Homecare Homebase (HCHB)
• Ensure documentation complies with Medicare Conditions of Participation, payer requirements, and agency standards
• Maintain compliance with HIPAA and all applicable privacy and security requirements
• Collaborate with clinical staff, including RNs, PTAs, OTs, STs, and leadership
• Participate in care coordination related to patient needs, visit completion, and regulatory requirements
• Maintain a current, unrestricted Colorado Physical Therapist license
• Maintain a valid driver’s license and reliable transportation for in-home patient visits
