Job Title: Case Manager
Location: Grand Junction, Colorado
Schedule: 8:00AM – 4:00PM x 5
Term: 8-week assignment with the potential for extension
Pay Rate: $49 – $53/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a Case Manager for a temporary assignment in Grand Junction, Colorado. These positions are 8-week contracts with the ability to extend.
Responsibilities for a Case Manager:
• Maintain and improve the case management system using ongoing interaction with patients, physicians, and other health providers to meet designated clinical, operational, and financial outcomes for aggregate patient populations.
• Ability to document Case Management plans in a clear and concise manner.
• Assist physicians, patients, families, staff and payers in providing care for patient populations that is appropriate, effective, and cost efficient.
• Attend and actively participate in all departmental and interdepartmental meetings relative to Case Management/Social Services functions.
• Collaborate with payers and review organizations to ensure proper utilization of patient care resources.
• Document clearly and concisely all contacts and information of the patient’s case management process appropriately.
• Educate physicians and staff regarding appropriate level of care/utilization issues.
• Maintain current knowledge of Case Management/Social Services, discharge planning, and nursing issues through continuing education.
• Suggest and introduce new methods and techniques.
• Participate in professional activities in order to continue professional development and contribute to that of the profession.
• Apply knowledge of the continuum of care, discharge planning, and utilization processes.
• Maintain current BLS certification.
• Maintain current unrestricted Registered Nurse license in the State of Colorado.
• Provide weekend coverage as required: 5 hours for one Saturday each month with corresponding time off during the week.
