Job Title: LCSW
Location: Albany, Oregon
Schedule: 8:00AM – 5:00PM x 5
Term: 13-week assignment with the potential for extension
Pay Rate: $48 – $52/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a LCSW for a temporary assignment in Albany, Oregon. These positions are 13-week contracts with the ability to extend.
Responsibilities for a LCSW:
• Provide social work services to hospice patients and family members through the final stages of illness
• Support patients in various home settings including personal residences, skilled nursing facilities, assisted living facilities, and adult foster homes
• Deliver medical social services and counseling as part of an interdisciplinary hospice care team
• Provide bereavement support to patients and families
• Coordinate and collaborate with hospice team members including nursing, spiritual support, hospice aides, and trained volunteers
• Maintain current Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) licensure
• Maintain current Healthcare Provider Level BLS certification
• Utilize personal automobile for patient visits and maintain a valid driver’s license with required auto liability/property damage insurance
• Maintain personal vehicle in good working order to meet job requirements
• Apply experience in hospital or medical social work settings to support patient care delivery
