Job Title: Adult-Gerontology Nurse Practitioner (AGNP)
Location: Escondido, California
Schedule: 8:00AM – 5:00PM x 5
Term: 26-week assignment with the potential for extension
Pay Rate: $111 – $120/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a Adult-Gerontology Nurse Practitioner (AGNP) for a temporary assignment in Escondido, California. These positions are 26-week contracts with the ability to extend.
Responsibilities for a Adult-Gerontology Nurse Practitioner (AGNP):
• Provide home-based hospice and palliative care as a Nurse Practitioner for patients with serious or life-limiting illnesses
• Conduct comprehensive patient assessments and develop individualized care plans
• Manage symptoms and provide goals-of-care discussions with patients and families
• Perform home visits, including advance care planning (POLST/advance directives)
• Provide patient and family education related to hospice and palliative care
• Coordinate care with the interdisciplinary team and community physicians
• Participate in rotating weekend coverage and occasional on-call duties as part of 24/7 patient coverage
• Document all patient encounters accurately in the EMR (My Unity)
• Participate in Interdisciplinary Team (IDT) meetings and quality initiatives
• Collaborate closely with the Hospice & Palliative Care Medical Director
• Complete approximately 45–50 patient visits per month
• Serve as a resource for home aides and the care team
• Maintain valid California NP & RN licenses, DEA, NPI, BLS, and AANP/ANCC certification
• Obtain ACHPN certification within one year of hire (preferred at hire)
• Maintain a valid California driver’s license and appropriate auto insurance for local travel to patient homes
• Spanish bilingual skills are a plus
