Location: Avondale, AZ
Schedule: 8:00AM – 5:00PM x 5
Term: 13-week assignment with the potential for extension
Pay Rate: $59 – $63/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a PT for a temporary assignment in Avondale, AZ. These positions are 13-week contracts with the ability to extend.
Responsibilities for a PT:
• Complete required documentation directly with ATI outside of standard clearance packet before being cleared to start assignment
• Must obtain permanent license immediately if starting with compact license (SC requires permanent license only, no compact)
• Work 32 guaranteed hours per week during holiday weeks
• Maintain active license throughout the assignment
• Provide proof of license application within 5 business days of booking
• Ensure start dates align with license status at time of submission
• Meet experience requirements specific to the assigned setting
• Adhere to day shift scheduling requirements
• Comply with overtime billing at 1.5x rate when applicable
• Provide 30 days notice as required per contract terms
• Utilize facility-provided equipment as outlined during interview process
• Follow dress code requirements as provided during first day orientation
• Adhere to facility policies and procedures for patient care delivery
• Candidates previously employed as perm/PRN staff must have been separated for one year before being eligible to return
