Job Title: PHARM
Location: Sitka, Alaska
Schedule: 7:00AM – 5:00PM x 4
Term: 13-week assignment with the potential for extension
Pay Rate: $86 – $92/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a PHARM for a temporary assignment in Sitka, Alaska. These positions are 13-week contracts with the ability to extend.
Responsibilities for a PHARM:
• Review all prescriptions for appropriateness of therapy, legality, completeness, contraindications, interactions, allergies, adverse drug reactions, and lab value abnormalities, including screening patient charts
• Provide clinical consultation and clarification, resolving any concerns or questions with the prescriber prior to dispensing
• Document all clinical activities and interventions accurately and completely
• Enter prescriptions into pharmacy software and verify finished prescription orders in a timely manner while maintaining all required record keeping and documentation
• Work independently with minimal supervision, organizing and prioritizing work assignments to ensure pharmacy services are provided in a timely manner
• Advise patients clearly and accurately regarding their medications, appropriate outcomes, drug name and strength, usage instructions, and expected changes, verifying patient understanding
• Communicate regularly with patients and healthcare providers to resolve problems and provide optimal care
• Perform chart reviews for prescription refills and medication therapy management, writing recommendations for providers and requesting refills on patients’ behalf
• Provide drug information, drug therapy consultation, and staff education relating to drug therapy
• Direct and verify the daily activities of pharmacy support personnel; serve as a role model and educator for pharmacy students and technicians
• Participate in clinical and administrative activities including quality control and quality improvement within the department
• Attend meetings and participate on committees as requested
• Assure availability, preparation, and control of medications and select appropriate drugs for formulary
• Work in pharmacy-run clinics and take on additional departmental duties and responsibilities as assigned
• Provide health promotion and disease prevention activities related to drug use and preventative drug therapy
• Maintain current Basic Life Support (BLS) certification for healthcare providers
• Maintain an unrestricted license as a registered pharmacist in the State of Alaska
• Maintain current flu vaccination
