Astrya Global, a San Diego based medical staffing agency, is hiring a Home Health for a permanent assignment in Wrangell, Alaska.

Responsibilities for a Home Health:

• Implement safe, therapeutic, and efficient care for patients and residents with needs due to multisystem disease and/or complications of treatment using appropriate infection control measures
• Set priorities for care of patients and residents based on acuity and/or patients’ preference; anticipate potential problems and adapt to changes in the area
• Initiate, manage, and document nursing procedures consistent with scientific principles and departmental policy
• Demonstrate knowledge of equipment use and accurately administer age-specific medications and therapeutics
• Carry out medical and nursing care plans and coordinate resident care activities with physicians and other healthcare team members
• Initiate action to reduce, correct, or prevent immediate, ongoing, or potential risks to the resident
• Facilitate healing by providing nursing care that is individualized, goal-oriented, and based on scientific principles and the nursing process
• Explain tests, procedures, and disease processes to residents and significant others
• Communicate and interact with residents and significant others in a positive and supportive way; provide counseling using emotional, intellectual, and psychological supports
• Evaluate residents’ progress or lack of progress towards goals, directing new goal setting and implementing revised care plans as directed by reassessment
• Document residents’ responses to care and maintain confidentiality of resident information
• Accomplish nursing assessment through interviewing, observation, and the nursing process; obtain, interpret, and apply age-related patient information
• Complete thorough Nursing Admission Databases and initial resident assessments; document assessment findings consistent with other providers
• Identify problems and nursing diagnoses by recognizing the impact of health problems on residents and families
• Perform duties as assigned and ensure quality resident care in both routine and emergent situations
• Impart educational knowledge to develop skills and enhance decision making by interacting with residents, family, staff, and community
• Organize and participate in care conferences as applicable; identify staff, resident, and family needs and develop plans for education or make appropriate referrals
• Develop and implement educational programs and learning aids in the department
• Provide discharge information and participate in in-services and educational programs as assigned
• Complete all mandatory training and competency checks
• Communicate with staff, residents, significant others, and other members of the healthcare team in a supportive, constructive, and positive manner
• Assist in the orientation of new staff, travelers, and students by sharing knowledge and experience
• Guide others in choosing nursing interventions, developing skills, and applying knowledge
• Follow and direct others to follow corporate, hospital, and departmental policies, procedures, and resident care standards
• Perform as Contact RN, leading assigned RNs, LPNs, and CNAs and accomplishing all assigned resident care and area support activities during the shift
• Develop and contribute to a plan of nursing action based on existing and potential resident problems
• Transcribe and note physician orders accurately; utilize past experience to provide a range of options for nursing care
• Initiate care plans using resident care standards and coordinate the plan with the healthcare team
• Write progress notes in the EHR and give shift reports according to area guidelines
• Attend and participate in at least 50% of departmental staff meetings and apply information presented
• Complete annual competencies, evaluation materials, and maintain certifications and licensure
• Comply with dress code and area and departmental scheduling guidelines
• Refer unresolved problems with patients, visitors, or co-workers to Nurse Manager or Administrative Manager for resolution
• Maintain familiarity and competency in mandatory state reporting guidelines and their time requirements
• When DON is not physically present, be willing to take the role of charge nurse for the long-term care unit
• Perform other duties as assigned
