Job Title: LCSW
Location: Albany, Oregon
Schedule: 8:00AM – 5:00PM x 5
Term: 13-week assignment with the potential for extension
Pay Rate: $48 – $52/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a LCSW for a temporary assignment in Albany, Oregon. These positions are 13-week contracts with the ability to extend.
Responsibilities for a LCSW:
• Provide social work services to patients and family members through the final stages of their illness
• Support patients in home settings including personal residences, skilled nursing facilities, assisted living facilities, and adult foster homes
• Assist with pain and symptom management coordination and counseling services
• Provide medical social services and spiritual support coordination
• Collaborate with interdisciplinary hospice team including nurses, hospice aides, volunteers, and bereavement support staff
• Maintain Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) licensure
• Maintain Healthcare Provider Level BLS certification
• Utilize personal automobile for patient visits across Linn, Benton, Marion, Polk, Lincoln, and Tillamook Counties
• Maintain a valid driver’s license and auto liability/property damage insurance as required by law
• Maintain personal vehicle in good working order
• Apply experience in hospital or medical social work settings to hospice care delivery
• Work toward or maintain Oregon LCSW (Licensed Clinical Social Worker) licensure preferred
