Job Title: Home Health
Location: Kremmling, Colorado
Schedule: 7:00PM – 7:00AM x 3
Term: 13-week assignment with the potential for extension
Pay Rate: $39 – $42/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a Home Health for a temporary assignment in Kremmling, Colorado. These positions are 13-week contracts with the ability to extend.
Responsibilities for a Home Health:
• Assess, plan, implement, and coordinate patient care for the Skilled Care Unit (SCU)
• Provide care to assigned patient populations in accordance with established standards
• Work a schedule of 36 hours per week consisting of 3 nights at 12 hours per shift
• Participate in on-call rotation, taking call for a minimum of 2 shifts per 6-week period
• Be available for potential overtime as needed
• Maintain a valid LPN license to practice in Colorado
• Maintain current BLS certification
• Provide possible coverage at additional care settings as needed
