Location: Lakewood, Washington
Schedule: 8:00AM – 5:00PM x 5
Term: 26-week assignment with the potential for extension
Pay Rate: $62 – $67/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a PT for a temporary assignment in Lakewood, Washington. These positions are 26-week contracts with the ability to extend.
Responsibilities for a PT:
• Provide physical therapy services as a PT contractor
• Maintain an active state license for the applicable region (WA and AK may work on a Compact License)
• Complete required credentialing documentation and contract QA requirements prior to start
• Complete additional documentation directly with ATI outside of standard clearance packet before being cleared to start assignment
• Commit to a 13 to 26-week assignment booking duration
• Work a minimum of 40 guaranteed hours per week, with 32 hours guaranteed during holiday weeks
• Work day shift hours as scheduled
• Adhere to facility dress code requirements as provided during first day orientation
• Utilize facility-provided equipment as directed
• Self-report any non-billable time as applicable
• Adhere to a 30-day notice requirement prior to end of assignment
• Comply with all facility policies and procedures for patient care delivery
• Disclose any previous work history at ATI prior to submission for eligibility review
