Job Title: Home Health-Hospice
Location: Barre, Vermont
Schedule: 7:00AM – 7:00PM x 3
Term: 20-week assignment with the potential for extension
Pay Rate: $62 – $67/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a Home Health-Hospice for a temporary assignment in Barre, Vermont. These positions are 20-week contracts with the ability to extend.
Responsibilities for a Home Health-Hospice:
• Provide individualized goal-oriented nursing care using the nursing process in accordance with a physician’s order
• Serve as Case Manager in cases where this position is the lead discipline
• Deliver exceptional home health and hospice care to individuals of all backgrounds
• Ensure access to high-quality services regardless of financial circumstances
• Provide holistic care including health promotion, long-term care, and maternal-and-child health services
• Work a full-time schedule, Monday through Friday
• Enrich the lives of community members through dedicated home health and hospice services
