Job Title: Hospice
Location: Missoula, Montana
Schedule: 8:00AM – 5:00PM x 5
Term: 8-week assignment with the potential for extension
Pay Rate: $55 – $59/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a Hospice for a temporary assignment in Missoula, Montana. These positions are 8-week contracts with the ability to extend.
Responsibilities for a Hospice:
• Provide skilled nursing and coordinate care to patients and family members in a residential setting through a team approach and based upon systematic evaluation as authorized by a physician
• Observe, assess, evaluate, and plan interventions in relation to patients’ physical and psychosocial statuses, including changes in condition and/or reactions to treatments or medications
• Oversee care provided by Home Health Aides and CNAs to assigned patients and evaluate compliance with plan of care
• Assure continuity of care through effective communication, information exchange and case conferences with other disciplines, community health, support and social agencies
• Provide quality care and appropriate symptom management for positive clinical outcomes and promote optimal health and quality of life through teaching, counseling, appropriate interventions and referrals
• Maintain accurate, current and complete patient records in accordance with agency standards and OASIS requirements as well as knowledge of current reimbursement rules and regulations
• Participate in team scheduling coordination and on-call rotation schedule as necessary to meet patient care needs by assessing and evaluating patient needs via telephone and advising or making home visits as required
• Participate in quality improvement activities and attend in-service training as assigned
• Maintain a current RN license in the State of Montana (BSN preferred)
• Maintain a valid driver’s license, appropriate auto insurance, and reliable transportation
• Utilize HCHB experience for documentation and patient record management
• Work Monday-Friday 8:00-5:00 with on-call and weekend rotation as required
