Job Title: PT
Location: Sunnyvale, California
Schedule: 8:00AM – 5:00PM x 5
Term: 13-week assignment with the potential for extension
Pay Rate: $65 – $70/hr
Astrya Global, a San Diego based medical staffing agency, is hiring a PT for a temporary assignment in Sunnyvale, California. These positions are 13-week contracts with the ability to extend.
Responsibilities for a PT:
• Restore function to prevent or minimize disability following disease or injury
• Help clients reach their maximum performance and function within the limits of their capabilities
• Evaluate and treat patients using medically prescribed physical therapy programs
• Supervise Physical Therapy Assistants as needed
• Conduct initial admission and evaluation visits
• Notify assigned Clinical Team Manager of OASIS scores within established timelines
• Document observations, clinical findings, problems, skilled interventions and treatments, goals, and discharge plans
• Perform duties consistent with skills and training for the Physical Therapist role
• Travel to and provide services within assigned territory and neighboring cities as needed
• Maintain a minimum of one year of Home Health experience
• Perform other duties as assigned in alignment with organizational goals and mission
